COVID-19 & Flu Vaccination Event

A parent or guardian must be present at the time of vaccination for children ages 12 - 17. The vaccine will be administered on Saturday, Oct. 22nd from 9 AM to 2 PM. Please fill out the registration form below (a separate form for each individual or child). The COVID-19 vaccine is free. The flu vaccine is free with insurance or $32 without insurance. Please bring your ID, previous vaccine card (if you have one), and your insurance card.
